The collection of fluid in the middle ear:

The collection of fluid in the middle air is a disease which is called serous otitis in medical terminology and observed at the back of eardrum without temperature and with the collection of fluid in the middle ear.

In whom is fluid collected in the middle ear and what are the symptoms?

The collection of fluid in the middle ear is found in most children. The most important symptom is weakening of hearing. As there is no any severe pain and temperature, one should be attentive in diagnose. Child’s increasing the sound on the TV, decrease in the reaction to sound, speaking out loud, or lagging in lessons when the grades are good can be related to weakening of hearing emerged with the collection of fluid in the middle ear.

How does the collection of fluid in the ear occur?

Children tend to inflammation of the middle ear. The middle ear is opened to the nose with Eustachian tube. Its main function is to provide ventilation and drainage of the middle ear. Eustachian tube is located horizontally in children in comparison with adults and it is formed as in adults at the age of 7. At the same time, mostly, along with adenoid vegetation causing congestion in Eustachian tube observed in childhood, it also comprises reservoir for microbes playing an important role in the inflammation of middle ear. Middle ear inflammation is mostly observed during the first 3 years old. There can be fluid in the middle ear in 40% of children who have middle ear inflammation with the probability of 1 month and in 10% of children with the probability of 3 months or more.

How is the fluid collected in the middle ear in adults?

It occurs mostly after upper respiratory tract infection which has viral origin. In some cases, it can be the result of a tumor located in nasopharynx (nose – pharynx). You have to be careful in such cases.

How is it diagnosed?

Turbidity in eardrum, transparency, reduction of air bubbles at the back of membrane, in some cases, collapse in eardrum can be observed. There can conducted tests for measuring pressure in middle ear and evaluation of hearing in accordance with the doctor’s advice in these cases.

How is the treatment carried out?

The treatment is started with drug treatment in most cases. According to the doctor’s decision, one or more of antibiotics, decongestant sprays, cortisone sprays and antihistamine drugs in allergic patients can be used. The patient is followed after this treatment. If the changes such as collapse in eardrum, reduction in hearing are not observed, the patient is controlled at intervals of up to 3 months. If the fluid is not sucked during this period, surgical operation is suggested.

How should be surgical treatment conducted?

Making cut in the eardrum, the fluid in the middle ear is drawn and the tube which is known as ventilation tube and ensures the ventilation of middle ear is placed in the ear. If the child has adenoid, it is removed in the same surgery.

What kind of anesthesia is used during the surgery?

It is carried out with general anesthesia in children patients. It can be conducted with local anesthesia in adults.

Is the tube is seen from outside?

No, it is not seen.

How long does the tube remain?

It is related to the type of the placed tube. Decision is made according to the type and problem of the tube which is placed. Mainly, it stays for 6 – 12 month and gets removed itself. It is seen that the tube is discharged into the road of external ear in the examination and it is taken by the doctor. If the tube is not discharged by itself and a period more than 1 year has passed, it is taken by the doctor.

What to be paid attention to after placing tube?

The ear should be protected from water. Cotton with Vaseline should be put into the outer ear passage after each washing. If inflammation or discharge is observed in the ear, then you should consult doctor. Infection is taken under control with the drops to be prescribed.
